I don't think it's happened since like, 1920? It led to the baseballs being replaced between each pitch and started a movement towards batting helmets.

In the 1970's(?) a player in the minors died from a NSFL collision in the infield. A player was wearing a metal knee brace that cut the other player. Yikes.

A pitcher getting killed from freak line drive shot off his temple is probably more likely. Kind of amazing it hasn't happened at least once...probably has in the minors or college or high school somewhere.
