Injuries like that can happen off of nothing if the process started. Like for a regular person if you consistently have sore achillies tendon issues or you step back and it hurts at times you should be very careful if you decide to play pickup basketball you could blow your achilles.

In all reality you could tear your achillies walking across the living room if you have damaged it, and fraying rope is a solid comparison.

I broke the scaffoid (navicular) bone which is the bone right next to the one they are discussing here. No joke. I had a cast on for 16 weeks and the X-ray said it still hadn't healed due to the low blood flow. Some days I'd never know I had ever broken it and some days it feels paralyzed for minutes at a time and I cant even hold a pencil.... usually depending on weather. Cold wet days are the worst.
